- 消化器症状：吐き気、食欲不振、軽度の嘔吐。
現場での迅速血液検査と血小板モニタリング
医師が30分以内に客室に到着後、直ちに以下の迅速検査を実施します：
- デングNS1抗原迅速検査：発熱の初期1〜5日間に高い感度を示し、15分以内にウイルスの非構造タンパク質1を検出します。
- デングIgM/IgG抗体検査：発熱が5日以上続く場合の二次免疫応答を評価します。
- 全血球計算（CBC）：白血球数（白血球減少症が一般的）およびヘマトクリット値を測定します。
- 血小板数の継続的監視：重症化の鍵となる血小板減少症（thrombocytopenia）の進行を追跡するため、危険期には毎日の検査が不可欠です。
客室での治療方針：水分補給と安全な対症療法
デング熱はウイルス性疾患であるため、特異的な抗ウイルス薬はありません。治療は細心の支持療法と体液バランスの維持に集中します：
- 経口および点滴による水分補給：経口補液塩（ORS）またはベッドサイドでの点滴（IV）により、適正な循環血漿量を維持します。
- 安全な解熱鎮痛薬の選択：パラセタモール（アセトアミノフェン）のみが推奨されます。アスピリン、イブプロフェン、ロキソプロフェンなどのNSAIDsは、血小板機能を阻害し重篤な出血リスクを高めるため絶対に服用しないでください。
- 継続的な経過観察：熱が下がり危険期に入る48時間は、医師が毎日訪問して血小板数を再検査し、全身状態を確認します。
直ちに救急搬送が必要なデング熱の危険兆候
発症後3〜7日目、熱が下がり始める時期に、重症デング熱（デング出血熱や血漿漏出）へ進行する危険期を迎えます。以下の警告サインが1つでも現れた場合は、直ちにカインホア省総合病院（19 Yersin）へ緊急搬送する必要があります：
- 激しく持続する腹痛または圧痛
- 持続的な嘔吐（24時間以内に3回以上）
- 粘膜からの出血（歯茎の出血、鼻血、吐血、黒色便）
- 極度の倦怠感、意識の混濁、激しい不穏状態
- ヘマトクリット値の上昇を伴う急激な血小板数の減少

顎や腕に放散する激しい胸痛、突然の重い呼吸困難、止まらない大出血、突然の意識喪失、痙攣、顔面のゆがみや片麻痺などの脳卒中徴候がある場合は、直ちに115に電話して公的救急車を要請するか、カインホア省総合病院（19 Yersin）または第87軍病院（19-21 Nguyễn Chánh）へ直行してください。往診サービスは命に関わらない急な体調不良に対応しています。
